Founded in Kenya in 2014, the Malindi Desire Initiative (MDI) began as a support network for transgender individuals living with HIV. Recognizing the urgent need for a structured approach to advocacy and service provision, the organization was officially registered in 2016 as an entity dedicated to supporting Intersex, Transgender, and Gender Non- Conforming (ITGNC) persons.
MDI was born from a fundamental question: “How far are you willing to go for a community you love?” This guiding principle has shaped its mission to empower and uplift transgender and gender non-conforming (TGNC) individuals who frequently encounter stigma, discrimination, violence, and barriers to healthcare. Through a holistic approach, MDI provides essential services, fosters safe spaces, and advocates for the rights and well-being of TGNC communities.
Beyond direct support, MDI is committed to driving societal change by engaging the broader community in conversations about the challenges faced by TGNC individuals. By fostering awareness, promoting inclusivity, and advocating for policy changes, MDI strives to build a more equitable and accepting society for all

MDI’s core focus areas encompass a holistic approach to community empowerment, centering on health, advocacy, and social inclusion.
- Health Promotion and Service Provision – MDI is dedicated to enhancing the well- being of Intersex, Transgender, and Gender Non-Conforming (ITGNC) individuals, as well as the broader LGBTQ+ community, by providing comprehensive healthcare services. This includes HIV prevention, testing, and treatment, mental health support, STI screening, and access to gender-affirming care. Through awareness campaigns, peer education, and community outreach, MDI promotes health-seeking behaviors and ensures that marginalized groups receive the care they need.
- Policy Advocacy – Recognizing the systemic challenges faced by ITGNC and LGBTQ+ individuals, MDI actively engages in policy advocacy to influence laws, policies, and institutional frameworks that impact their rights and access to This involves collaborating with stakeholders, engaging in dialogue with policymakers, and working to eliminate legal and social barriers that contribute to discrimination and exclusion.
- Religious and Faith Restoration – Many ITGNC and LGBTQ+ individuals experience rejection and alienation from religious institutions due to their gender identity and sexual orientation. Target Population: MDI’s primary target population includes Intersex, Transgender, and Gender Non-Conforming (ITGNC) individuals, as well as other key and vulnerable populations who face significant social, economic, and health disparities.
- Intersex, Transgender, and Gender Non-Conforming (ITGNC) Individuals – MDI prioritizes the needs of ITGNC individuals, who often encounter systemic discrimination, violence, and barriers to essential These challenges make it difficult for them to access healthcare, employment, legal recognition, and social acceptance. MDI works to address these gaps by providing tailored health services, advocacy, and safe spaces for community support.
- Key Populations – This includes groups that are disproportionately affected by health inequities, especially in relation to HIV and other sexually transmitted infections (STIs). These populations include gay, bisexual, and other men who have sex with men (GBMSM), sex workers, and people who inject drugs. Due to stigma, criminalization, and restrictive policies, key populations often struggle to access healthcare and legal protection, making MDI’s work crucial in ensuring their rights and well-being.
- Vulnerable Populations – These are individuals and groups who experience heightened risks due to economic hardship, social exclusion, or cultural factors. This may include queer individuals who have been rejected by their families, those facing homelessness, survivors of gender-based violence, and individuals in crisis MDI provides support by offering community-based interventions, mental health services, economic empowerment programs, and advocacy for their rights.
Achievements & Impact
MDI has achieved significant milestones in advancing healthcare access, research, and advocacy for Intersex, Transgender, and Gender Non-Conforming (ITGNC) individuals and other key and vulnerable populations.
- Integration of Key Population Health Services within a Public Health Facility – MDI made history by establishing its office within the Malindi Sub-County Hospital, making it the first queer-led organization in Kenya to be fully integrated into a government healthcare facility. This integration has allowed MDI to provide comprehensive, stigma-free health services—including HIV prevention, testing, treatment, mental health support, and gender-affirming care—directly within a public health setting. This unique model bridges the gap between key populations and mainstream healthcare, ensuring that ITGNC individuals and other marginalized groups receive quality care without fear of discrimination.
- Collaboration with KEMRI and Published Research – MDI has worked closely with the Kenya Medical Research Institute (KEMRI) on various research projects, contributing to national and regional discussions on LGBTQ+ health, HIV prevention strategies, and gender-affirming care. Through this partnership, MDI has actively participated in scientific studies and published articles that provide critical data on the health needs and challenges faced by ITGNC individuals and other key populations in Kenya. This research has helped inform policy recommendations and improve healthcare programming for marginalized communities.
