The landscape of HIV prevention has fundamentally changed. The days of relying solely on one or two methods are over. Today, we are witnessing a revolution in biomedical and community driven options, and it is crucial that every sex worker across Africa knows that we have a right to access them.

This isn’t just about new drugs or medical technology it’s about a radical shift in philosophy. It’s about recognizing that our lives, our working conditions, and our bodies are all different. What works for one person might not work for another. One size does not fit all.

The Power of Choice

True empowerment begins with knowing your options. This means understanding that prevention isn’t just about a single action in a single moment. It is a range of tools that can be tailored to fit your specific reality.

Some people prefer a predictable routine, making daily oral PrEP (Pre Exposure Prophylaxis) a reliable constant. For others, whose sexual activity is irregular or requires complete discretion, the emerging options for event driven (2 1 1) PrEP or long acting injectables (like Cabotegravir or the newly released  Lenacapavir) provide flexible, powerful protection that fits their lives. The choice of a is  logistical, personal, and sometimes, a matter of safety and privacy.

The choice of monthly vaginal rings or consistent condom use (both male and female) empowers individuals to maintain control over their health, often without needing partner consent. This type of autonomy is vital.

A Rounded View of Health

Real prevention goes beyond simply avoiding an infection. It means caring for our entire selves. This is why voluntary self testing, routine STI screening, and treatment are non negotiable pillars of health. They normalize sexual health maintenance, remove stigma, and allow us to be proactive.

Similarly, we cannot speak about comprehensive prevention without acknowledging the intersecting needs of harm reduction, especially needle and syringe programs. We refuse to exclude any member of our community based on their practices. Harm reduction is prevention, and it is a fundamental right.

Crucially, the “Power of Choice” must also extend to our reproductive futures. This means robust support for the Prevention of Mother to Child Transmission (PMTCT). A positive HIV diagnosis should never be a barrier to planning and having a healthy family. Effective treatment during pregnancy means a nearly 100% chance of an HIV free baby. Reproductive justice and HIV prevention are linked.

U=U

Finally, our menu of choices is fortified by the transformative concept of U=U: Undetectable = Untransmittable. This is perhaps one of the most powerful tools in our collection. When people living with HIV are on effective anti retroviral therapy (TasP) and maintain an undetectable viral load, they cannot pass the virus to their sexual partners. U=U destroys stigma and enables us to build supportive populations

In Conclusion.

Sex workers deserve every option available. We deserve healthcare that is respectful, comprehensive, and non judgmental. We deserve to choose what fits our lives, our work, and our reality.

We will not settle for “good enough.” We demand access, we demand knowledge, and we celebrate the strength of our community.The power of choice is ours. My Health. My Safety. My Choice.
